All students to whom I serve as examiner, reviewer, supervisor must sign a copy of this form and submit to me (in person, by snail mail, or via the General Office), before I can start reading your report/thesis/dissertation. In short: Before you give me any of your report to read, sign this form and hand it in to me. No signed declaration, no review. Please read FAQ on plagarism (search "plagarism" on this site). Also read the examples posted outside my office.
